Call (502) 935-8061 and we will schedule your appointment for the first available opening. Due to unexpected emergencies, it’s possible that we may need to change your appointment. If you need to cancel or reschedule your appointment, please notify us as far in advance as possible so that your time slot may be given to another patient in need of care. A parent or legal guardian must accompany patients under age 18.
You will be asked to provide the following information when you make your appointment:
If this visit is related to a motor vehicle accident or an injury covered by workers’ compensation, we will need to verify your insurance information before you are seen. Please call our office with this information at least two days before your appointment so we can pursue preapproval. Doing so will help avoid a delay or the need to reschedule. If you were injured in a motor vehicle accident and a referral is required, please get one as soon as possible. This is necessary in case your auto insurance benefits run out; otherwise you will be responsible for medical costs.
If you need to reach a physician when our office is closed, call the office at (502) 935-8061 and our 24-hour answering service will forward your message to the physician on call.
If you have an emergency, call 911 or go directly to a hospital emergency department. Tell emergency department staff the name of the physician who is treating you for your condition.
All new prescriptions and refill requests must be completed by phone or online during normal office hours. For refills, you may use our automated pharmacy refill line, (502) 935-8061, ext. 1280. Requests received after 3 p.m. will be processed the next business day. We cannot refill prescriptions outside of office hours.
The Prescription Agreement is a contract between you and your physician. This contract is an agreement to prevent any misunderstandings about prescription medicines you might take for pain control while being treated by this medical practice.
Please understand that if you break this contract, your physician will stop prescribing pain control medications for you. In addition, your physician reserves the right to refuse to prescribe pain control medications to you in the future and to discontinue seeing you as a patient if you break the contract.
We will complete your forms for disability or other insurance claims. We offer this service to all our patients, and forms are completed in the order in which they are received. To accommodate all our patients’ needs, we ask that you turn in the forms as soon as possible. You should allow five to seven business days for your forms to be completed. There is a charge for completing these forms. Please ask about this fee when your turn in your forms.
At times, you may need to obtain a copy of your medical records. To ensure your confidentiality, you will be asked to sign a release of medical records form before we allow you or anyone else to access your medical information. We cannot release records with a telephone request. We must have a signed release from you, the patient. Fees may apply. Be sure to check when requesting records.
