Thanks to earlier detection, enhanced treatment and ongoing support, survival rates are improving for individuals who have been diagnosed with cancer. In fact, there are more than 11 million cancer survivors in the United States alone.

Cancer patients receive a large amount of information, and establishing a survivorship care plan allows for optimal long-term care. It provides both patients and caregivers with the opportunity to understand the potential for future effects related to the cancer or treatment.

The care plan may include:

• Treatments and medications
• Frequency of follow-up appointments with a medical provider
• Follow-up tests and the frequency they are required
• Doctor or other health care professionals needed for follow-up care
• Information about how to be proactive in monitoring for relapse/recurrence or developing a second primary malignancy
• Signs or symptoms to watch for
• Who to call in case of emergency
• Common long-term side effects of treatment
• How to maintain health and well-being
• Support groups and survivorship programs
• Information about employment and health insurance
